She built the business from a solo brick-and-mortar practice in Austin into a 16,000-patient network with 10 clinicians and three physicians in leadership — all self-funded and with zero marketing spend. Her team uses proprietary lab ranges and treatment protocols, with documented remission of Hashimoto’s disease. Dan Bova: Can you give us the elevator pitch for Modern Thyroid Clinic?
McCall McPherson: Modern Thyroid Clinic started as a brick-and-mortar medical practice in Austin, Texas. The demand was so large that a few years ago we expanded nationwide. We serve women and men, but many of our patients are women with complex thyroid disorders who cannot get the help and care they need within the traditional medical landscape, which remains quite antiquated for thyroid conditions. Modern Thyroid Clinic offers these patients a chance not only to regain their lives and thrive but also to improve long-term health outcomes and prevent chronic disease.
From Personal Struggle to Professional Mission
What inspired you to start it?
At 27, while practicing as a Physician Assistant (PA), I was battling a long-term thyroid condition that left me bedridden for 16 hours a day. When I sought medical help, my doctor ran a single thyroid panel and dismissed my symptoms, advising me to “eat less, exercise more” and prescribing cholesterol medication at an unusually young age. Frustrated but determined, I joined a waitlist to see a physician who approached thyroid health more progressively. This doctor ordered a comprehensive thyroid panel that examined six markers rather than just one. The results were clear: my condition was treatable with a different class of medication, one that less than 10% of Americans receive for thyroid issues. This treatment changed my life profoundly and inspired the creation of Modern Thyroid Clinic.
The Challenge of Thyroid Treatment Today
Why is it so hard to get treatment in the first place?
The medical system often oversimplifies hypothyroidism. Physicians typically have about eight minutes per patient, making it difficult to address complex, non-procedural medical issues adequately. Standard practice involves running a single lab test and, if results appear normal, reassuring the patient they’re fine or prescribing a single class of medication offered to roughly 91% of thyroid patients. If symptoms persist, the assumption is that the problem lies elsewhere. However, this one-size-fits-all approach fails many patients—as with cholesterol medications, if a treatment is ineffective, it needs to be adjusted. This crucial flexibility is often missing in thyroid care.
Innovating Care: A New Business Model for Better Patient Outcomes
How did you build a business model that solved the time problem?
We dedicate an hour to each patient. Instead of rushing through brief consultations, we spend sufficient time to inform, empower, and educate patients, guiding them through a thorough diagnostic and treatment process. While we don’t accept insurance for the initial consult, we do for labs and medications. This exchange of time for dollars has proven successful and allows us to uphold high standards of care.
Scaling from Solo Practice to Nationwide Network
You started as a solo clinician. What’s the story from “one office” to where you are now?
I began as a one-woman operation in a single office while raising two daughters, one of whom is disabled. This personal responsibility fueled my determination. By focusing on a niche that delivers rapid, tangible improvements, patients returned and naturally spread the word. Without spending a dollar on marketing, our waitlist grew to 9,000 women seeking care. That momentum led to launching a national telehealth network that now serves 16,000 patients, supported by a medical team of 10 clinicians and three physicians in leadership—all of whom have personal experience with thyroid-related health issues.
Practical Advice for Those Struggling to Feel Well
If people don’t feel good and they don’t know why, what are the first steps they can take?
I strongly advocate for data-driven health decisions. The first step is to get a full thyroid panel and understand the results using our proprietary optimal lab ranges, which show where labs should ideally be. For men, it’s important to check testosterone levels—both free and total. Finding a healthcare partner who looks beyond the standard eight-minute consultation and the “normal” lab ranges is essential. Sometimes, paying out of pocket is an investment worth making because every dollar spent on health can yield a hundredfold return in productivity. For entrepreneurs especially, investing in health is the highest ROI, as poor health undermines performance and outcomes.
Exploring GLP-1 Microdosing: A New Frontier in Metabolic Health
You’re also a leading voice on GLP-1 microdosing. What is that exactly?
GLP-1 microdosing involves administering doses much lower than the traditionally prescribed minimum. Many side effects reported in media are linked to excessive dosing combined with insufficient food intake. By microdosing—using doses as low as one-eighth or one-quarter of the lowest available dose—we achieve weight loss benefits with minimal or no side effects. Moreover, GLP-1 medications have been documented to provide significant longevity benefits. Our lab data confirm that these microdoses deliver similar positive effects as standard doses.
Dispelling Common Health Myths
What do you wish people would stop repeating as health advice?
Two major misconceptions persist. First, the outdated “calories-in versus calories-out” model assumes everyone has equal biology, which is false. Many people have metabolic dysfunctions—thyroid issues, hormonal imbalances, pre-diabetes—that cannot be resolved simply by lifestyle changes alone.
Second, there is an unfounded fear linking GLP-1 medications to thyroid cancer. The black box warning originates from rat studies that used doses 20 to 100 times higher than human equivalents, which led to medullary thyroid carcinoma in rats. However, humans do not share the same thyroid receptor patterns, and human studies have consistently shown that GLP-1 medications do not cause thyroid cancer.
